Kishi did say someone was going to end up blind, and someone was going to end up unable to fight ever again.. so Kakashi's likely ONE of those 2 people, though I guess they kind of go hand in hand. Kakashi losing his shinobi status is the same as him dying in my opinion.

You go blind if you dont have EMS, whether kakashi has EMS or not is unknown, but hes only used it...like...three times...itachi has used it known 6 times, and many times before that most likely, and his sight was 50% deteriorated, so kakashi shouldnt go totally blind yet.

Because sasuke is weak compared to itachi, and his sharingan is pathetic.

itachi used it LOADS, and we saw in the manga that his sight was "liney" on sasuke before his tsukyomi was beaten.

I think the after effects of mangekyou hurts, as zetsu and sasuke stated itachi was slower and more blind directly AFTER tsukyomi, and must get better after a while, so the immediate effects appear to be quite nasty, longterm takes some time

However thats pure conjecture on my part.
