Do you have to have Tsukuyomi and Amaterasu to unlock it? Or do you just have to have unlocked both abilities in your eyes?

It doesnt have to be tsukoyomi cause sasuke doesnt have it but still has susanoo. It is said that those who have awakened ms in both of their eyes can have susanoo so kakashi cant have it. In one of sasukes eyes its amaterasu and the other can manipulate its form or can put it out as he recognised it during the fight with hachibi.
