Appearently it doesn't come out of mouth, only magically appears near your mouth. Kakashi did the same thing with Katon: Goukakyuu in the anime.

The only confusing element, is we've not seen Tobi use the mangekyo to this point. Why would Kakashi need to use MS, but Tobi doesn't, if they're truly connected like that?
