Tsuande uses a jutsu in order to make herself look older, right? And while she was on the brink of her death during pein arc the jutsu wore off because she used too much chakra or something like that. So if she happens to die, we might be able to see her true old granny face instead of the current younger one.

If she does die, I don't think Kishi would tarnish her character like that. It would be the last image of her we had and it would probably be the image we conjure up when we think about her. What has been seen cannot be unseen.

I have great respect for Tsunade.. only because of her medical prowess and strength, however i think if people saw her real face, it wouldn't look that bad right?... well if you saw Jiraiya.. he didnt look like Sarutobi age or anything...(i know he was younger but anyway..). Orochimaru cannot be counted as he is just freaky in general..... But her large chest will no longer be there.. sad times for Tsunade :/
