He used Hado 96 against Aizen. That's why he doesn't have his arm anymore. Hado 96 requires the user to sacrifice one of their limbs.

You cannot heal that it is from the sacrificial hadou #96 the arm is lost for ever it requires one sacrifices their limb.

I guess in the sacrifice of the hand, the hand was basically "sealed" (imo, pretty much like edo tensei in naruto, sealed dead people cant come back to live, sealed arm cant come back to body)

The reason yama-jii doesnt want the hand healed is because he likes remaining with scars of his previous battles..e.g the scars he got after fighting his vice captains bankai etc
