If he had died old, he would have taken over when Tobirama Died, as Hiruzen did when Minato died, but instead tobirama left hiruzen in power after his death.. he had to have died some time before then. He probably sacraficed himself at some point as this seems like the pattern. .. the possibilities are endless, but untill kishi says something, we will never know.

Hashirama had an extremely intense battle with madara that probably left him permanently damaged from jutsus like his healing tech or external damage issued by madara. Also it is very possible he was swarmed by many shinobi in this war or was put in a similar situation like tobirama where he was outnumbered or maybe a combo of the two.

I dont think ounumbering would help if he can easily change landscapes :D Most likely he got overrun by horde of ninjas and run out of chakra.

the most likely person to have killed hashimara is madara, i mean it has been said that hashimara died on the first war, and madara and hashimara were both still alive during this war so i think that madara went back for revenge and killed him. i dont see why madara wouldnt of go back and get revenge. this is the most likely way that hashimara died

As others have said, the first databook (page 116) attributes his death as being due to "one of the many raging wars that were raging during this revolutionary era."

And honestly I think that's the better cause of death. It shows that no shinobi is unbeatable or immune to death.
