Uhm... Maybe Karate Shoukoushi Kohinata Minoru. About a guy that quits gymnastics and joins the Full Contact Karate Club at his University.
Also Initial D.About street racing and whatnot.

At the moment, the most realistic I can think of is Usagi Drop. It's about as slice of life as I've ever gotten, but it's interesting in that the idea of single parenting is one that many anime really don't seem to have (or focus on even if it is there).
Clannad (supernatural elements aside) and Toradora are pretty realistic too though if only because I feel like I can relate to the feelings, thoughts, and struggles of the characters and really connected with them throughout.

I'd say Hana Yori Dango. It's about a girl who enters an elite High School and is immediately picked on by a group of four boys who call themselves the F4 (Flower four). This group leaves a red card with the initials F4 in the locker of their next victim. So when the rest of the school finds out one of the students has the ''red card'' in their locker they immediately start bullying this one person until that person is forced to leave the school, and in the worst of cases is forced to commit suicide.
It's in my opinion a very well-built manga/anime (I only read the manga) as you can see how the protagonist has the will to deal with all the bullying and is even able to fight her way back to the school. And not to mention that the bad guy ends up falling for her but she wil give him the hardest of times as a payback for what he did to her.

realistic as in artwork, or story?
I can think of many who would fit in any of the two types, some have already been mentioned: Usagi drop is a good example, also thinking of ergo proxy in terms of design.... but I am not sure what you meant
