It may have already been pointed out, but go back and look at the manga chapters here. Itachi is shown outside his Susanoo multiple times.

he's basically inside it there. but when itachi struck nagato with the totsuka the three of them were standing to the side and susanoo was standing next to itachi a way off, it wasnt like that in the manga for sure.

He's not inside it, itachi came sliding from elsewhere after susanno already cut nagato.. thus his feet has a sliding motion on it but susanno is at a standstill

Itachi is where bee/naruto at, aka at the hands of Susanno.. which is on the OUTside of the entity.. not the inside

omg...who cares, its not ruining the story. And besides i think its cooler that you dont always have to be direcly under susanoo. Actually i just realized that its already proven that susanoo can be maintained while on the outside so theres no real fail here
