if he existed in the manga timeframe there would have been a medic around to heal his injury. its implied he slowly died either of infection or blood loss since he had enough time and energy to rip his own eyes out. tsunade saved COUNTLESS lives by instilling the medical divisions and squads. which other nations copied (shee, chiyo). what was mortal back then is easily patched up in 4th world war

What i don't get about the timeline revolving around izuna is was madara going blind in his battles against the senju or is that all irrelevant because before it was stated izuna gave his eyes to madara because madara was losing his light now its implying izuna gave his eyes to madara because he was dying.

Anyway this one actually makes sense. Izuna probably would've lived if someone was there to heal his wound. Although we don't know the extent of his damage either, and medics can't heal all injuries. So we still don't really know, but he would've had a better chance of living atleast. Either way, it was Tobirama who gave the wounding blow, no escaping that
