Think about it he died and disappeared in other words probably into hiding but what i think shodaime did to defeat him was remove his eye and thats why he probily wears that mask because you see itachi meat him and he still has the mask on either that or Narutos dad did it to defeat him at the appearance of the nine tails . and it also explains why hes a shell of his former self hes only at half the power
well thats my theory i fell what happened to his eye

and also the reason he wanted the uchias dead is for the truce they made with the leaf village that is why assisted itachi and he trained itachi so he could become useful for attaining the kyubi but he realized itachi was to strong for him so he figured to have sasuke but he knew itachi wouldn't allow it so long as he lived so he probably bided his time and corrupted sasuke at his weakest point to get him on his side which we all know and i reckon madra will disown akatuski after he has all the tailed beasts hes using them to obtain them because he hasnt got the strength he used to seein as he lost his eye so he used everyone as leverage to get them

I think that's a very good theory. Possibly, he wants to use the Bijuu's power to use a form of space/time ninjutsu to recover his eye and restore his power.

Although, when he took off his mask enough to see his "good eye", there were no signs of bandages or anything. He does wear some form of headband, we were able to see that much but no more :/

I mean, chronologically, that would make a ton of sense.
He got into the fight with Shodaime and looks regular, with two eyes, then after the fight, he disappears.
After THAT he somehow becomes the Mizukage.
Than AFTER THAT he re-appears, and suddenly has a mask covering his face and exposing one eye.
???
The only thing that could be wrong with that is the fact that Kisame didn't recognize him with the mask on, if he had the mask on while he was Mizukage, Kisame would have known right away.
BUT Madara has been shown to change his masks as well, and he has also been shown to change his hair, so maybe that's why he didn't recognize him?

Maybe his eye is still hidden in Konoha somewhere 0_o
